Could an open door or window be the cause?

Yes, and it's one of the most overlooked issues. Every time an exterior basement door or window opens, especially on a humid day, it introduces air with a much higher moisture content than the conditioned space. A single hour of an open window on a 90% RH summer day can undo hours of dehumidifier work. Weatherstripping gaps, an unsealed crawlspace access hatch, and a dryer vent leaking into the basement are common hidden sources too.
